He was predeceased by: his parents, John E. Forstrom and Hazel E. Forstrom (Swinderman); his son David Forstrom; and his siblings, Ben Forstrom, Robert Forstrom, Jack Forstrom and Ruth Strictmaker.
He is survived by: his wife Barbara L. Wise; his children, Lynn Ann Keener (Mark) of Dover and Michael J. Forstrom (Cindy) of Cincinnati; and his grandchildren, David Keener (Cindy) of Dover, Eric Keener (Rachel) of Dublin and Kara Lynn Rodgers (Jeremy) of New Philadelphia.
