He is survived by: his parents, Melody Lewis (Banal) and John Lewis; his daughter Destanie Lewis; his siblings, Dennis, Michael Donley, Amber Paulsen (Lee) and Brittany Mozingo; and his uncle Mark Lewis. He is also survived by several cousins and several nieces and nephews.
