He was predeceased by: his parents, Laverne Crooks and Donna Crooks (Tuller) of Sunbury, Ohio.; his grandparents, Frances (Brookens), Orville Tuller, Eric and Elodia Crooks (Heil); and his nephew Peter Leaf.
He is survived by: his siblings, Terry (Julie Crooks), Greg (Deborah Leaf) and Richard (Lisa Crooks); his aunt Joyce (Tuller); and also, Alicia and Bob Seitz. He is also survived by many nieces, nephews.
In lieu of flowers, memorial contributions may be made to any local Hospice.
