He was predeceased by: his parents, John M. Herbert and Florence Sylvia Mullen. He is survived by: his children, Danielle Cozad (Jonathan), John Herbert (Sarah) and Benjamin Herbert; his grandchildren, Austin Cozad and Addilyn Cozad; his siblings, Rita Schuhart and James Herbert; and his pet Shadow. He is also survived by two nephews and a niece, cousins, an uncle.
