He was predeceased by: his father Phil. He is survived by: his wife Cathy; his sons, Micah (Debi) and Jeff (Jordan); and his grandchildren, Jackson, Ryan, Evie, Elliot and Sam. He is also survived by siblings and extended family.
In lieu of flowers, the family requests that donations be made in Phil's name to St. Jude's Children's Hospital.
