He was predeceased by: his parents, Eugene D. Kolakowski and Mary S. Kolakowski (Galanzosky); and his sisters, Mary Ann and Marilyn. He is survived by: his siblings, Larry (Joyce) of California, Ed (Rocio) of Spain and Clare Kolakowski of Oregon. He is also survived by nieces, nephews, and other family and friends.
