He was predeceased by: his father Glenn Cozad; his sons, Eric Cozad and Aaron "Scoby" Cozad; and his brothers, Lynn Cozad and Terry Cozad.
He is survived by: his wife Nancy Cozad (Fisher); his daughter-in-law Geaine Cozad; his stepson John Shade, Jr. (Amanda); and his grandchildren, Jocelyn Shade and Trevor Shade.
