He was predeceased by: his parents, Clarence Guy and Olive McMahan Guy; his former wife Estella Staab Guy (Toni); his wife Maribelle Barr Guy; his son Greg Guy; and his siblings, Frank Galbreath and Monna Yassemi.

He is survived by: his children, Geri Rubicky (Rob) of Columbiana, Jeffery Guy (Jeanette) of Greenford, Randy Guy (Kim) of Columbiana and Jack Guy, Jr. (Amber) of Columbiana; and his grandchildren, Daniel, David, Michael, Jason, Jonathon Guy, Justina Stryffeler, Jared Channell, Kristen Snyder, Cory Guy, Greg, Mark Fitzsimmons, Stacie Perez, Tyler and Ryan Guy. He is also survived by 12 great grandchildren.

Memorial donations may be made to the Columbiana Stadium Restoration Fund or the Columbiana 40 & 8.
