Chambers of Clyde and Brian Albert Chambers (Kelly) of Perrysburg; his grandchildren, Britney Chambers, Dakota Chambers, Morgan Chambers, Zachary Chambers and Blake Chambers; his great grandchildren, Aiden Zelms and Aubrey Zelms; his siblings, Jack Chambers and Judy Hansen; and also Rickie Wenrich of Clyde.
Memorials may be made, if desired, to a favorite charity of the donors in Jerry's name.
