He is survived by: his wife Dianna Maynard; his children, Leonard Amos Perkins IV, Melissa Ann Perkins, Bonnie Michelle Perkins, Josh Smith and Kenny Maynard; his grandchildren, Jacob Perkins, Bailey Borgia, Emily Borgia, Jaxxon Smith and Lukka Smith; and his sister Sheila Perkins.
