He was predeceased by: his parents, Jessie Smith, and Harold Smith, Sr.; and his son Johnny Shumar. He is survived by: his children, David Harley Carroll (Tabitha), Michelle Martincic (Jack) and Kristen Stasek; and his sisters, Carol Hollows (Michael) and Alice Smith (John Guadalupe Flores II). He is also survived by 10 grandchildren.
