She was predeceased by: her child Brian Cooke; her parent Eulala Church; and her sibling Glenn Rice. She is survived by: her spouse Roger L. Weaver; her stepchildren, Laura Cooke, Brendan O. Weaver and Audrey V. Weaver; her parent Roger Church; her parents-in-law, Thomas and Virginia Weaver; her sibling Melissa L. Griswold (Michael); her siblings-in-law, Debbie, Jeffrey Burtch, Tom and Pam Weaver; and her sibling Donna Lewis. She is also survived by 9 nieces and nephews, special friend.
Donations to the Brian and Kenneth Cooke Memorial Fund c/o Morehead State University Foundation Inc. Mindy was employed at the University of Cincinnati Medical Center for 32 years before retiring in 2016 and graduated from Morehead State University in 1978.
