He was predeceased by: his parents, John Sr. Mental and Addie Mental; his sisters, Barb and Geraldine; and his brothers-in-law, Mike and George.

He is survived by: his wife Kathy; his children, John III (Andrea) and Jaclyn (Jeff); his grandchildren, Connor, Matthew and Olivia; his siblings, Peggy, Tom (Laurie) and Mickey; and his brother-in-law Bill Kuhn (Cindy). He is also survived by many loving nieces and nephews.
