He was predeceased by: his son Anthony F. Mlakar Jr.; and his parents, Anthony Marie and Anna Marie. He is survived by: his wife Rita; his children, Judith Hayman (Frank Shattuck), Bob Mlakar (Theresa Battaglia), Kathleen Mlakar-Cook (Robert) and Ann Marie Mlakar-Petroff (James); his grandchildren, Eric (Laurie), Michael Madsen (Lauren), David Mlakar (Alicia), Nicole Mlakar, Samantha Rubino (Miguel) and Jeanette Blakeway (Kurt); his great grandchildren, Alyssa, Madelyn, Olivia, Vivian, Nathan Madsen, Josephina, Luciana Mlakar and Jay Blakeway; and his sisters, Janet Mlakar (late Frank) and Dorothy Bianchi (late John).

In lieu of flowers contributions may be made in memory of Anthony to St. Jude Children's Research Hospital.
