He was predeceased by: his mother Mary Coughlin; his daughter Mary Andzelik; and his sister Mary Kiggins. He is survived by: his wife Carolyn Hampton Andzelik; his children, Jerome Andzelik III of Brunswick, Tammy Blaylock of Spencer and Jessica Gossman of Dresden; and his siblings, Larry Andzelik of Fla, David Andzelik of Solon, Tommy Andzelik of Solon, Patty Leman of Pennsylvania, Rita Copley of Pennsylvania, Dolly Andzelik of Pennsylvania and Debbie Golick of Pennsylvania. He is also survived by 10 grand Children and several great grandchildren.
