He was predeceased by: his parents, Donald G Forsyth and Frances Bradshaw Forsyth; his brothers, Jessie, Terry and Bill; and his parents-in-law, Pauline Smith and Maywood Smith.
He is survived by: his daughter Amanda Forsyth; his grandchildren, Dakin Chadwell and Sevon Altop; his brother Dan Forsyth (Tracie); and his siblings-in-law, Linda Forsyth, Penny Forsyth, Joyce Stutler (Don), Lisa Dickens (Guard) and David Smith (Theresa). He is also survived by several nieces and nephews.
In lieu of flowers, contributions may be made to the funeral home for expenses.
