He was predeceased by: his parents, Gerald "Pete" Chadwick and Irene Chadwick (Smith); his siblings, Martha Kirk and Dale "Squirrel" Bernard; and his daughter Amy Bernard.

He is survived by: his wife Margie; his children, Melinda Bernard, Laura (Barrett), Mike and Joey; his grandchildren, Brayden, Gavin, Dylan, Gaige, Layla, Lola and Sandra; and his siblings, Greg Chadwick (Holly), Rita Longshore (Jim) and Mabel Engert (Scott). He is also survived by numerous nieces, nephews, and cousins.
