She was predeceased by: her parents, George Forgach and Sally Forgach (Zaleski). She is survived by: her brother David (Rosemary); her nephew Tim (Tara); and her great-nephews, Caleb and Cole Forgach.
In lieu of flowers, memorial donations in Monica's name may be made to St. Paul Catholic Church of North Canton.
