He was predeceased by: his parents, Gaylord Cook and Ruth Cook (Craft); his sister Jean Freier; and his parents-in-law, Fred and Charlotte Miller.

He is survived by: his wife Carol Miller; and his sons, Gary W. Cook (Kathy), Robert S. Cook (Laura) and William F. Cook (Cheryl). He is also survived by ten grandchildren and twelve great grandchildren.

In lieu of flowers memorial contributions may be sent to Hospice of the Valley or Angels for Animals.
