He was predeceased by: his parents, Stephen Kotasek and Susan Kotasek (Goske); his wife Norene Carole Tablack; his siblings, Stephen, Lawrence, Gerald, Virginia Horvath, Susan Drapp, Margaret, Florence, Thomas, Raymond, Charles, Robert, Joseph and Mary; and his father-in-law Andrew "Bucky" Tablack.
He is survived by: his daughters, Pamela and Denise; and his sister Patricia of Arizona.
