Cheryl J Forman Obituary

With solemn hearts, we announce the passing of Cheryl J Forman of Cambridge, Ohio, whose presence will be deeply missed, having departed on September 11, 2024 at the age of 71. Leave a sympathy message to the family on the memorial page of Cheryl J Forman to pay them a last tribute.

She was predeceased by: her mother Bonnie Bakos (Anker); and her grandparents, Frank, Velma Anker, Mike and Mary Bakos.

She is survived by: her father Michael Bakos; her husband David Forman; her sons, Isaac Forman (Christy) of Strasburg, VA and Ethan Forman (Heather) of Cambridge; her brother Jim Bakos (Paula) of Byesville; her grandchildren, Ezra, Archer, Ellarose, Josie and Silas; her nephews, Chris Bakos (Julie) and Matt Bakos (Nicole); and her great-nephews, Braeden, Peyton, Nick and Tony.

Visitation was held on Tuesday, September 17th 2024 from 4:00 PM to 7:00 PM at the Thorn-Black Funeral Home Inc. (139 S 9th St, Cambridge, OH 43725). A Mass of Christian burial was held on Wednesday, September 18th 2024 at 11:00 AM at the Christ Our Light Catholic Church (701 Gomber Ave, Cambridge, OH 43725).
