He was predeceased by: his parents, Frederick Smith and Miriam Smith (Patton); and his brother Paul A. Smith. He is survived by: his wife Dez; his daughter Holly Ortiz (Mark) of Columbus; his grandchildren, Emma Farmer (Chad) of Cincinnati, Joey Ortiz of Westerville, Gabe and Brady of Columbus; his great grandchildren, Pearson and Lakyn Farmer; and his siblings, Nancy Allamon (Tim) of Bucyrus and Mike Smith of Crestline.
