He was predeceased by: his parents, John L. Cook and Edna M. Cook (Wetmore); his daughter Lori Ann Brown; his stepson Richard Shirkey; and his brothers, John Cook and Joe Nisley.
He is survived by: his daughter Amy Jo Whitcomb of Fort Wayne; and his stepson Dan Shirkey of Bryan. He is also survived by 3 grandchildren.
