He was predeceased by: his parents, Frank Dvorak and Cora Mae Dvorak; and his brother David Frank Dvorak (Barbara). He is survived by: his wife Barbara A. Dvorak; his children, Anne E. Davis, Robert S. Dvorak (Maria) and Donna L. Bate; his grandchildren, Scott Dvorak, Benjamin Dvorak (Emily), Madalyn Wojda (Matt) and Brian Davis; his great grandchildren, Evelyn Grace Dvorak and Charlotte Louise Dvorak; and his sister-in-law Gloria Zielenski (late Richard). He is also survived by 5 nephews and 1 niece.
In lieu of flowers, the family suggests memorial contributions be made to a charity of choice.
