He was predeceased by: his parents, James Cizek and Nettie Cizek; and his brother James Cizek. He is survived by: his children, David Cizek, Darlene Unetic (Monte), Robert Cizek and Frank Cizek (Lisa); and his grandchildren, Amy, Shaun, Jamie, Kevin and Frank.
