He was predeceased by: his parents, John Allen Church and Rosalee Opal Church (Rosencrance); and his brother Richard Church. He is survived by: his wife Paula J. Church of Braceville; his children, Felicia Church (Earl Hoschar) of Deerfield, OH and James (Sara Church); his siblings, Linda Swick of Champion, Barb Allison of Champion, Debbie (John Dale), John Church Jr. of Hubbard, Bill (Tonya Church), Chuck (Cindy Church) and Shawn (Kim Difford); and his grandchildren, Kaydence, Tyler, Lillian, Miles and Lilah.
