He was predeceased by: his parents, Dimitri "Mickey" and Mary Sawayda Slogik; his daughter Theodora "Teddy" Booze; his son-in-law Troy W. Booze; and his brother George Slogik.
He is survived by: his wife Carolyn Rose; his daughter Tamara Middleton (Tim) of Warren; and his brother Steve Slogik (Gloria) of Altoona, PA. He is also survived by four grandchildren.
In lieu of flowers, the family suggests that memorial contributions take the form of donations in Joe's name to Crossroads Hospice.
