He is survived by: his wife Karen Johnson; his daughters, Erin Clark (Kyle) of Bowling Green and Elizabeth Michalec (Christopher) of Winston Salem; his grandsons, William and Wesley Clark; and his sister Terry Ely (Dennis) of Lanesville, Indiana..

Memorial contributions may be given in his memory to the Alzheimer's Association.
