He is survived by: his great grandchild Avery Johnson; his children, Jimmy Dale Patrick, Jr. (Kimberly) of Leavittsville and Tonya Bigler (Bill) of Sherrodsville, OH; his siblings, Wiley J. Patrick of Bolivar, OH and Virginia Fowler of Zoarville, OH; and his grandchildren, Jami Patrick, Jonathon Patrick and Brandon Johnson. He is also survived by his family and many friends.
