He was predeceased by: his parents, Dean W. Polen and Gloria Hedge Polen; and his brother-in-law Randy Heaston. He is survived by: his siblings, Dean W. Polen, Jr. (Sherri) of St. Clairsville, Cathy Heaston of Hopedale and Martha Bethel (Richard) of Maineville, Ohio; his nieces and nephews, Jenna Tipton (John), Kelley Polen, Adam Heaston and John Polen; and his great-nephew Kyler Dean Polen.
