He is survived by: his parents, Hubert Perkins and Suzan Wise Perkins; his fiancée Misty Maynard; his daughter Shelby Leann Perkins; his stepchildren, Kristin Milstead and Johnathan Maynard; his brother Jesse Perkins; and his grandchildren, Chelsea, Caden, Tori and Landon Milstead.
