He was predeceased by: his parents, Peter Collins and Jessie Pearl Boothby (Hughes); and his siblings, Johnny, David Boothby, Delores Duncanson and Jane Ann Ballein.
He is survived by: his wife Dorothy Ann Boothby (Glover); his children, Shiela McGuffey (Tom Friedhoff) of Batavia, Ohio, Connie Krebs (Shelby) of Batavia, Ohio and Chuck Boothby (Amy Taylor) of Batavia, Ohio; and his siblings, Gene Boothby (Sandy Mitchell) of Mt. Orab, Tulane Barber (Bill) of Mt. Orab and Darlene Murrell (Jim) of Dayton, Ohio.. He is also survived by eleven grandchildren; three great grandchildren.
