He was predeceased by: his father Keith Shultz, Sr.; his grandparents, Lloyd, Dema Shultz of Belmont, Elijah and Marjorie Kerr of Brazil, Indiana; his uncle Thomas Flight of Cookeville, Tennessee; and the mother of his children Keli Phillips of Ohio.
He is survived by: his mother Deborah Kerr Shultz (Wayne) of Zanesville, Ohio; his children, Kayla Daugherty (Joshua) of Ohio and Kyle Shultz of Indiana; his grandchildren, Bayleigh, Ryon, Jayden and Kamdyn; his brother Jason Shultz (Leslie) of Barnesville Ohio; and his aunts, Evelyn Flight of Tennessee, Rebecca Murphy (Richard) of Indiana and Bernice Chase (Jerry). He is also survived by several nieces and cousins.
