He was predeceased by: his parents, Phillip M. Smith and Bette Jean Warner Smith (Blair); and his brothers, Frank Smith, Gary Smith and Keith Smith.
He is survived by: his daughter Amanda Smith of Bellefontaine; his brothers, Steven Smith of Lakeview, Jerry Smith of Apopka, FL, Gregory Smith of West Liberty and Mark Smith (Tammy) of Bellefontaine; and his pet Cat. He is also survived by numerous nieces and nephews.
