He was predeceased by: his parents, Arthur P. Cullen and Emily L. Cullen (Kaahaaina); his daughter Ruth A. K. Cullen; and his siblings, Ryan Cullen and Lenora Kealoha-Cullen.
He is survived by: his wife Ruth A. Cullen (Tanaka); his children, Corrine Cullen and David Cullen, Jr. of Bellefontaine; and his siblings, Carolyn Machado of California, Melanie Cullen of Hawaii and Abraham Cullen of Arizona. He is also survived by numerous nieces and nephews.
