He is survived by: his parents, Dale Johnson (Deb) of Bellefontaine and Bonnie Horsley of Bellefontaine; his daughter Jacqualine "Mackie" Johnson of Bellevue, Ohio; his sisters, Tracy Johnson of Bellefontaine, Vanessa Rollins (Jamey) of Bellefontaine, Mindy Johnson of Bellefontaine, Kasi Johnson of Bellefontaine, Kari Johnson of Bellefontaine and Angel Moore (Bill) of North Lewisburg; and his nieces and nephews, Tiffany, Tabitha, Savanna, Katie, Whitney (Dustin), Mercedes, Zoey, Nick, Taylor (Emily), Blaize, Gage (Aryn), Treyce, Karsyn, Kaesyn, Sammy and Ryder.
