He was predeceased by: his parents, John Kapak and Vivian Kapak (Palmer); and his close friend Donnie West. He is survived by: his cousins, Paula Spence and Mike Bayham; his close friend Anna Foltz (Mike Alfrey); and his aunt Betty Prystasz. He is also survived by several other cousins and friends.
