She is survived by: her husband Bill Terry Carr; her sons, Dennis Phillips Jr. (Gladine) of Las Vegas, NV and Anthony Phillips of Austintown; her grandchildren, Amanda, Anthony Jr. and Andrew Phillips; her stepchildren, Mark A. Carr of Fort Myers, FL and Terri Carr of Courtland; her stepgrandson Wayne Davis; and her sisters, Ruth Ann Connelly of Girard and Karen Purnell of Austintown.
In lieu of flowers the family requests donations be made to MVI Hospice or the Ohltown United Methodist Church in Susan's name.
