He was predeceased by: his parents, Fred Perry and Dorothy Perry (Mortimer); and his sisters, Gail Urichich, Pam Eaton and Mary Jane Hively.
He is survived by: his wife Linda Perry (Carr); his children, Neil Perry (Maran) and Andrea Worrell (Scott); his grandchildren, Claire, Corra, Cru Perry and Vinnie Worrell; and his siblings, Nancy Martucci (Tony), Candy Estok and Troy Perry. He is also survived by many nieces, nephews and friends.
In lieu of flowers the family requests that donations be made to Hospice of the Valley in Dale's memory.
