He was predeceased by: his father Donald Cook Sr.. He is survived by: his significant other Christina Reese; his children, Ashley Cook, Alison LaGreca (Josie), Kaitlyn Cook, Amy Roberts, Heather Delaney (Matt), Ross Offill, Lindsay Offill, Zachary Cannon-Reese (Megan) and Garrett Hunter; his grandchildren, Bentley LaGreca, Emma Roberts, Harper, Jax Delaney, Bobby Offill, Luke, Landon, Laynie Offill and Avara Cannon-Reese; his mother Deborah Roberts; and his siblings, Donald Cook Jr., Kenneth, Keith Eads, Josh Cook, Krista Cook, Tracy Cook and Shell Cook.
