He was predeceased by: his parent Cheong Y Lee; and his grandfather John J. Bloom, Jr.. He is survived by: his mother Vickie Lee (Bloom) of Ashtabula; his brother John Kollarik of Cleveland; his grandparents, LaVaughn Bloom of Ashtabula and Tom Luce of Ashtabula; his aunts, Julie Aldrich (Jim) of Mantua and Janice Patterson (Richard) of Geneva; and his uncle Jim Bloom (Tracy Tyson) of Ashtabula.
