He was predeceased by: his parents, E. Duane Sample and Lilly Belle Sample; and his siblings, Nancy Burkhart (Delbert), Randolph and Mary Jane Sample.

He is survived by: his wife Sharon Sample of Ashland; and his daughter Natalie Sample of Ashland.

Visitation was held on Tuesday, January 14th 2025 from 4:00 PM to 7:00 PM at the Denbow-Gasche Funeral Home (313 Center St, Ashland, OH 44805). A funeral service was held on Wednesday, January 15th 2025 at 11:00 AM at the Trinity Lutheran Church (508 Center St, Ashland, OH 44805).
