He was predeceased by: his parents, Guy Mack and June Mack (Harper); and his sister Mary Beth Mack. He is survived by: his wife Kathey Mack (Ramsey); his daughters, Carrie Fanello (John) of Lucas and Kelly Royster of La Grange, KY; his grandchildren, Andrew Fanello, Anthony Fanello, Emma Guest and Brady Guest; and his brother Terry Mack (Judy) of Ashland.
