He was predeceased by: his parents, Russell "Cody" Burdette and Virgilia Burdette; his son Jerry Hendrix; and his brothers, Wayne and Duane Burdette.
He is survived by: his wife Phyllis Burdette of Ashland; his children, Denise Burdette of Worthington, Debbie Albert (Luke) of College Station, Darcy Shafer (Eric) of Worthington, Barb Hendrix of Ashland and Jon Hendrix (Angie) of Sullivan; his grandchildren, Katelyn, Elizabeth, Nathan Albert, Allyson, Caroline Shafer, Brandon Hendrix (Caity), Seth and Jordan Hendrix; his great grandchildren, Sophie, Chance and Willow Hendrix; and his sister-in-law Lotte Burdette of Westerville.
In lieu of flowers, memorial contributions may be made to a charity of donor's choice in loving memory of Doug.
