He was predeceased by: his parents, Charlie Mulkey and Sylvia Mulkey; his stepfathers, Frank Piatt and Howard Kochheiser; his grandsons, Dakota Mulkey and Dustin Wilson; and his siblings, Sharon Martin and Dale Piatt.
He is survived by: his wife Diane Mulkey of Ashland; his children, Tammy Hales (Jeremi Spade), Julie Mulkey (Serena) and David Mulkey (April); his grandchildren, C. J. Hales, Garrett Hales (Tiffani), Brett Mulkey, Taylinn Hales, Mikala, Kiley, Cara and Andrew Spade; his great grandchildren, Belle, Blaiden, Kendall, Willow, Kennedy and Haisley Hales; and his siblings, Charlotte Martin (Don), Emma Patterson and Daniel Mulkey (Ronda). He is also survived by numerous nieces and nephews.
