He was predeceased by: his father Emilio Leyva; his siblings, Robert and Irma; and his granddaughter Arianna Alvarado. He is survived by: his daughter Maxine Leyva (Adrian Alvarado) of Archbold; his stepson Eric Gomez (Stacey) of Maryland; his siblings, Amy Rice (Todd) of Wauseon, Margarita of Defiance and Danny Leyva (Delia) of Archbold; his grandchildren, Elijah and Cayden Alvarado; and his nieces and nephews, Alexi, Ava, Gabriela, Stephanie, Daniella, Jacob, Brandy and Daniel.
In lieu of flowers, the family asks that memorial contributions be made to the benevolence of the family.
