He was predeceased by: his parents, Leslie Midlam and Minnie Midlam (Ulmer); his wife Hazel Midlam (Patrick); his children, Jerry, Larry Midlam, Scott Mildam and Kenneta Lockwood; and his siblings, Marguerite Reese, Gladys Earhart, Marie House, Nellie Hicks, Irene Midlam and Lavern Midlam.

He is survived by: his grandchildren, Alydra Kelly Konopik (Karel), Nick Klein and Ryan Midlam (Krissy); his great grandchildren, Jorden Midlam, Luke Midlam, Kai Konopik and Vanessa Stewart; and his great great grandchildren, Iris Midlam, Cayden and Axel. He is also survived by numerous nieces and nephews.
