He was predeceased by: his parents, Jim Shook and Mary Shook (Frantz); his wife Kay L. Shook (Speckman); his children, Douglas and Kimberly Shook; and his sister Joan Lawson.

He is survived by: his children, Tammy, Mark Cromwell of Vandalia, Jeff, Anita Shook of Rossburg, Brad and Jennifer Shook of Ansonia; his grandchildren, Justin, Jalynn Cromwell, Kristin Cromwell (Seth Kennedy), James, Andrea Shook, Amanda Shook, Ross Shook, Mitchell Shook, Marissa Shook, Michael Shook and Mason Shook; his great grandchildren, Rylee Shook, Austynn Cromwell and Axyle Cromwell; his brother Max; and his siblings-in-law, Jo Ann Shook of Kettering and Paul Lawson of Florida.
