He was predeceased by: his daughter Alexis Marie Hoffman. He is survived by: his father James Joseph Hoffman of Austintown, Ohio; his wife Chris; his children, Haley A. Hoffman (John Calabrese) of Austintown, James J. Hoffman (Kennedy Allen) of Austintown and Dale G. Simpson of Zelienople, Pennsylvania; his mother Sandra Hoffman (Williams); his grandson Carson James Calabrese; his brothers, David Hoffman (Shannon) of Austintown, Ohio and Gary Hoffman (Brandy) of Austintown, Ohio; his aunt Nancy Johnson (James) of Austintown, Ohio; his niece Courtney; and his nephews, David and Rylen.
In lieu of flowers memorial contributions may be made to St. Jude's Hospital.
