He was predeceased by: his parents, William Lewis and Ruth Lewis. He is survived by: his wife Gwendolyn "Gwen" Card-Lewis; his children, LaKeesha Schouster of Brookpark, Shannon Lewis of Amherst and Mark D. Lewis Jr. of Amherst; his grandchildren, Daniel, Jelaina, Xiamara and Olivia; his sister Sandra Farley of Wakeman; and his niece Hannah Farley.
